New Patient Forms


To fill out these forms, please log on to the patient portal. You should see a "Fill Questionnaire" button within the appointment box on your dashboard. Click it, and make sure to complete BOTH folders. If you do not see the Fill Questionnaire button, please contact our office IMMEDIATELY at ncrader@texaschildneurology.com.

Then, click the "My Account" tab on the left of the page, and then select "Step One". After you complete Step One, select "Step Two" and complete it. Make sure to update Step One and Step Two if any of that information changes in the future.

If the patient portal is down, or you cannot get in contact with us, please download and fill out the following paperwork before coming in for your appointment. You can print out the completed forms and bring them to your appointment, or you can email them in advance to: ncrader@texaschildneurology.com.
