Cancellations

We require a 24 hour notice if you are unable to make your appointment. After two failed appointments, your physician may release you from his/her care. If you are over 15 minutes late, we may have to reschedule your appointment.


Financial Policy

We collect patient co-pays and/or deductibles prior to your seeing the physician. Please be prepared to pay. We accept cash, check, and card (MasterCard, Visa, American Express, and Discover).


Medical Insurance

Your insurance may not cover the full cost of your charges. Regardless of insurance, payment remains your personal responsibility and is due before you see the physician. We do not file secondary insurance.


Forms and Letters

There is a $20.00 fee for forms and letters completed or certified by the physicians. The fee is collected just prior to the release of the form or letter. Blank forms will not be accepted, and the parent portion must be filled out before being submitted to the physician. We require a three (3) business day turnaround time for forms and a one (1) week turnaround time for letters.


Medical Records

We charge $10.00 for the first 20 pages and $.25 for each page thereafter.  There is no charge for records that are sent  to a licensed health care provider, insurance company, or state agency. The fee is collected prior to receipt of the records. When requesting medical records, please contact our office at least five (5) days prior to the date the records must be received.


Phone Information

Please be advised that all calls for a physician will be returned within 48 hours based on the urgency of the call, per the physician. All EEG, MRI/CT, and lab results will be available after 48 hours. Chromosome test results will be available after 12-16 weeks. Sleep Studies will be available after one week. When leaving a message for the nurse/physician, please leave all pertinent information such as:

* The patient’s first and last name, with spelling.
* The patient’s date of birth.
* The patient’s physician.
* A detailed message regarding any questions or concerns you may have.
* A return phone number where you may be reached.

There are email addresses, which are preferred, available for the Certified Medical Assistants (CMAs) which allow for more detailed messages to be forwarded to a physician. Please allow sufficient time for a return call from a physician before repeat calls are initiated. We look forward to assisting you and your child.
