Cancellation

We require a 24 hour notice if you are unable to make your appointment. After two failed appointments, your physician may release you from his/her care. If you are over 15 minutes late, we may have to reschedule your appointment.

Forms and Letters

There is no fee for forms and letters of reasonable length to be completed or certified by the physicians. Blank forms will not be accepted, and the parent portion must be filled out before being submitted to the physician. We require a three (3) business day turnaround time for forms and a one (1) week turnaround time for letters.

Financial Policy

We collect patient co-pays and/or deductibles prior to your seeing the physician. Please be prepared to pay. We accept cash, check, and card (MasterCard, Visa, American Express, and Discover).

Medical Records

We charge $10.00 for the first 20 pages and $.25 for each page thereafter. There is no charge for records that are sent to a licensed health care provider, insurance company, or state agency. The fee is collected prior to receipt of the records. When requesting medical records, please contact our office at least five (5) days prior to the date the records must be received. Medical Insurance

Your insurance may not cover the full cost of your charges. Regardless of insurance, payment remains your personal responsibility and is due before you see the physician. We do not file secondary insurance.

Phone Calls

Please be advised that all calls for a physician will be returned within 1-2 business days based on the urgency of the call, per the physician. All EEG, MRI/CT, and lab results will be available after 2-3 business days. Ambulatory EEGs will take 10-14 days to result. Chromosome test results will be available after 12-16 weeks. Sleep Studies will be available after 2-3 weeks.
